Abernathy Home Care was founded in 1992 by Pamela Abernathy with the goal of helping seniors stay independent at home and providing meaningful employment opportunities to our community members.
Today, our agency is still family-owned and operated, and we are still putting our clients first in everything we do.

We are also very proud of our long history of providing trustworthy, reliable elder care through the Aging Services Access Points (ASAP) in Massachusetts and serving as a senior care resource for families in need of compassionate care.
Meet Our Leadership Team
Matthew Abernathy, President
As the son of Abernathy Home Care’s original founder, Matthew has quite literally grown up in the home care business. His work as a homemaker and chore worker further opened his eyes to the extensive needs of seniors in our area. After graduating from the University of New Hampshire with a Bachelor’s degree in Communications Matt pursued his passions for the outdoors, working as a Snowboard school manager and trainer at several large resorts. Returning to his roots in home care Matt spent 20 years as Operations Manager for the family business.
Matt’s long history in management has developed his recognition of being a good employer and providing quality home care services. Matt has participated in several Massachusetts Office of Executive Office of Elder Affairs (EOEA) pilot projects, including Electronic Visit verification (EVV), and is currently serving on the Executive committee of the Home Care Aide council.
As the owner of Abernathy Home Care, Matthew is focused on creating care that is centered on respect and dignity with a personal touch, as well as offering meaningful and rewarding employment opportunities for caregivers in our area. In his spare time, Matthew enjoys spending time with his family in the great outdoors, biking or snowboarding.
Sarah Clark, Agency Director
Sarah got her start in home care as a CNA, caring personally for seniors. Her compassionate nature makes her not only a spectacular caregiver, but an ideal manager as well. Sarah’s experience in hands-on care gives her a unique insight into the needs of both clients and home care aides. Sarah is able to communicate with clients, their family members, and our care staff with a knowledge, grace, and respect that helps everyone feel heard, seen, and well cared for.
In her spare time, Sarah enjoys spending time with her family and playing chess. She is also a skilled archer and marksman.
Lisa Furtado, Director of Operations
Lisa brings decades of experience in caregiving to her role as Director of Operations. She became a Certified Nursing Assistant (CNA) over 25 years ago to gain the skills needed to care for her own mother who was living with dementia and Parkinson’s disease. Since that time, Lisa has worked in a variety of roles in the care field, ranging from a Scheduling Coordinator to Senior Director of Operations. Both her professional and personal journeys have provided her with the unique skills needed to advocate for Abernathy Home Care’s clients and to be a resource for everyone we serve.
Lisa is dedicated to finding compassion-based solutions that improve the quality of life for every client we care for. Lisa’s personal mission statement is simple: Do what is right, not what is easy, and everything else will fall in place.
