Abernathy Home Care was founded in 1992 by Pamela Abernathy to help seniors stay independent at home and offer meaningful jobs to community members.
Today, our agency is still family-owned and operated, and continues to put our clients first in everything we do.

Meet Our Leadership Team
Matthew Abernathy, President
As the son of Abernathy Home Care’s founder, Matthew has quite literally grown up in the home care business. His work as a homemaker and chore worker further opened his eyes to the extensive needs of seniors. After graduating from the University of New Hampshire with a Bachelor’s in Communications, Matt pursued his passion for the outdoors, managing and training at several large snowboard schools. Returning to his roots in home care, Matt spent 20 years as Operations Manager for the family business.
Matt’s history in management shaped his commitment to being a good employer and providing quality home care services. He has taken part in several Massachusetts Executive Office of Elder Affairs (EOEA) projects, including Electronic Visit Verification (EVV). He currently serves on the Executive Committee of the Home Care Aide Council.
As owner of Abernathy Home Care, Matthew focuses on delivering respectful, meaningful, and personal care while creating rewarding jobs for local caregivers. In his spare time, Matthew enjoys being outdoors with his family, biking, and snowboarding.
Kim LaDuke, Vice President of Operations
With over 20 years of experience in home care and a background as a Registered Nurse (RN), Kim LaDuke serves as Vice President of Abernathy Home Care. Throughout her career, Kim has built and streamlined systems that enhance client and staff experiences while keeping operations seamless and care quality high.
As Vice President, Kim drives growth and operational excellence with an emphasis on quality, efficiency, and team empowerment. She develops systems that improve care delivery while supporting the professional growth of staff members. Her leadership philosophy emphasizes continuous improvement and transparency. This also includes giving each team member the tools and support they need to succeed.
Kim’s personal mission is rooted in the belief that businesses thrive when they prioritize people—both clients and employees. She also fosters a culture built on mutual respect and trust. She cultivates a work environment where innovation and collaboration flourish at every level.
Outside of work, Kim enjoys exploring wellness and mobility and spending time with her husband and family. She also gives back to the boxing community through her volunteer roles with GIRL Boxing Club and as a NH State Boxing Commissioner.
Lisa Furtado, Director of Operations
Lisa brings decades of experience in caregiving to her role as Director of Operations. She became a Certified Nursing Assistant (CNA) over 25 years ago to gain the skills needed to care for her mother, who had dementia and Parkinson’s disease. Since that time, Lisa has worked in a variety of roles in the care field, ranging from a Scheduling Coordinator to Senior Director of Operations. Her professional and personal journey gives her the unique skills to advocate for clients and support everyone Abernathy Home Care serves.
Lisa is dedicated to finding compassion-based solutions that improve the quality of life for every client we care for. Lisa’s personal mission statement is simple: Do what is right, not what is easy, and everything else will fall in place.
Melissa Swan, Nursing Manager
Melissa’s lifelong passion for helping others led her to pursue a Bachelor of Science degree in Nursing from Northeastern University in Boston. While there, she was also a member of Sigma Theta Tau, the international honors society for nursing. After graduation, Melissa began her work as an RN in outpatient hemodialysis before realizing her calling as a home health nurse.
As Abernathy Home Care’s Nursing Manager, Melissa supervises our home health aides in the field. She ensures that detailed care plans are created and followed to provide exceptional client care. Melissa is dedicated to creating an environment of safety, well-being, and improved quality of life for our patients and their families.

- We employ experienced home health aides (HHAs), CNAs, personal care attendants (PCAs), homemakers, and companion caregivers who are passionate for caring for others on a personal level.
- Ensure that all our caregivers have the skills they need to provide exceptional care. We provide our team with continuing education as well as ongoing training and support.
- We use Care Academy, a state-of-the-art online training platform that allows caregivers to take classes whenever and wherever is convenient for them.
- We also provide RN oversight to ensure quality care at all times.
- We perform thorough criminal background, Office of Inspector General, reference, and driving checks on all aides to protect the safety and wellbeing of all our clients.
- Our caregivers are covered with general liability and workman’s compensation insurance for your security and peace of mind.
